• Routine appointment hours are Mon-Fri, 9:30 AM to 2:30 PM
• Follow up appointments are scheduled for 45 to 50 minutes. 
  
• If additional time is needed (if available), there will be an additional fee.
• Clients are responsible for the full appointment fee (explained below) unless the appointment is rescheduled or cancelled MORE than 24 hours before the scheduled start time. 
• In the event of a medical emergency the appointment fee will be waived.
• There is a 15 minute grace period for beginning an appointment late after which the appointment is considered to be missed and the full fee applies.
• Here is an explanation of the full appointment fee. If paying privately, the full appointment fee is equal to the private payment amount for that appointment. If using an eligible insurance policy, the full appointment fee is equal to the total amount allowable as determined by the insurance plan.
